Formerly known as the Diabetes Care Program, is one of the largest and most comprehensive diabetes and endocrine programs in the country. The multidisciplinary approach to diabetes, endocrine and metabolic disorders combine clinical care, treatment, education, nutrition counseling and management for gestational, Type 1 and Type 2 diabetes.

Do I Qualify?
Employees, spouses and dependents enrolled in the organization's traditional health plan are eligible to enroll. This does not include enrollees of the high deductible plan at this time. This program is available to help employees enrolled in the Traditional Health Plan (PPO) , and dependents who have diabetes to maintain a standard of care and improve their health.

Your Wellness Incentive
Thrive participants must remain active in the program and adhere to all program requirements through 10/30 of the applicable incentive year, as well as have been active in the program for a minimum of 3 months. Points are awarded a maximum of one program for the applicable incentive year.
This program is worth 20 points if all requirements are met by self-reporting in WebMD under the "Weight Management" category. Requirements include completing 3 months (90 days) enrolled in the Diabetes Care Program before November 30th of the incentive year.
